Population Health Data Manager The Community HealthCare Association of the Dakotas (CHAD) is looking for an individual that can help us turn a wealth of data into the information health centers need to improve care. A successful candidate for the Population Health Data Manger role will have the skill to analyze and interpret health data, provide statistical support, and engage members in ways to effectively use and understand data generated through our Data Aggregation and Analytics System (DAAS). Population Health Data Manager role This position is responsible for supporting our member health centers in North Dakota, South Dakota, and Wyoming through the work of the Great Plains Health Data Networks (GPHDN) by assisting with the adoption and utilization of data using the DAAS to make operational decisions and improve outcomes. The DAAS is a population health management tool that gathers health center level data and allows for dynamic reporting at the health center and network level. Through daily management of the DAAS and dissemination of accurate data to our internal and external partners we can leverage health information technology (HIT) and data to ensure our members are delivering high-quality, culturally competent, equitable and comprehensive primary care to the patients they serve. Qualifications: Bachelor’s degree in a statistics, health information technology, or informatics related field or equivalent combination of education and experience in health information technology, informatics, or data analytics/reporting. Three or more years of experience in health information technology, informatics, or related experience. Experience with or similar population health data aggregation and analytics platform. * Experience in data analytics/reporting required. Familiar with relevant healthcare concepts, practices, and procedures. Experience and knowledge with electronic health records (EHR). Experience with statistical methods and models used in analyzing health care data and in data mining. Must have well-developed skills in group facilitation. Ability to travel to meetings and conferences with the 3 states we serve, regionally and nationally.
