Overview Bluestone Physician Services is a mobile primary care practice offering a unique and robust model of care; customizing care that is preventive, proactive, and includes all members of the care team, patients and their families. Join us in our team-based care as a Nurse Practitioner or Physician Assistant as we deliver best-in-class primary care to those with intellectual and physical disabilities. Our providers have deep knowledge of the unique aspects of Group Home settings of care. This role will serve patients that reside in group homes in the east and northeast area of Minneapolis/St. Paul such as Maplewood and White Bear Lake. Position Description Provide on-site, team-oriented primary care to patients within the group home setting living communities Full-time providers care for patients Monday-Thursday; Fridays are either clinical or an administrative day depending on provider communities and schedule. Manage a panel of 150+ patients (approximately 8-14 patients per/day) Ability to design and manage your schedule Develop and implement care plans appropriate to patients and their desires Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers Access to diagnostics such as labs and x-rays as needed Collaborate with community partners within home care, hospice, and other resources as needed Qualifications Graduate of NP program or PA program from an accredited school Must have and maintain a valid Minnesota License Professional clinical experience in primary care Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid drivers license and minimum state required liability auto insurance; local travel is required Demonstrated compatibility with Bluestones mission and operating philosophies Demonstrated ability to read, write, speak, and understand the English language Travel throughout Twin Cities Metro required Preferred Qualifications 2+ years of clinical experience and/or group home experience in another role Why Bluestone?
